Please read over the application carefully before answering any of the questions. Every question must be answered completely. Incomplete applications will be returned. |

Reference forms: Please be sure to sign the "Release" at the top of the page on every reference form before giving it to your reference source. The information provided by the reference will be considered confidential and disclosed only to those persons involved in the admissions process, namely the Executive Director and Certification Board of Governors.
The individual completing the reference form must mail all reference forms directly to the GCBAA.

A company's application, along with the supporting documentation, must be completed before a company representative will be permitted to sit for the examination and be interviewed by the Certification Board of Governors. |

Please include a check in the amount of $250 with the application. The application fee is nonrefundable and includes the examination fee. Checks should be made out to "GCBAA" or "Golf Course Builders Association of America."
IMPORTANT NOTE
The representative from your company that signs the certification application and takes the exam must be an officer of the company.
The Certification Exam will be administered at the GCBAA Annual Meeting held in February in conjunction with the Golf Industry Show and at the Summer Meeting in August in conjunction with the PGA Championship.
In order for a Certification applicant to be eligible to take the exam, all application and reference materials must be received at the GCBAA office at least sixty (60) days in advance of the start of that meeting.
Interviews of Certification applicants will be conducted at the discretion of the Certification Board of Governors.
